About Vital Care
Vital Care Elmhurst is a licensed infusion pharmacy with USP 797- compliant sterile
compounding facility. We provide specialty infusion medications, nutritional support, infusion pumps and
medical supplies. We are supported by the resources of Vital Care Elmhurst inc, which is providing high-tech
infusion services throughout the United States with over 70 locations.
Our staff provides comprehensive infusion therapy management to patients in their own homes and our
state of-the-art infusion suites. As part of our management service, we also act as a liaison to
physicians. We also provide free home delivery.

History
Vital Care Elmhurst,is established in 2023 to provide a comprehensive scope of
high-quality parenteral and enteral therapies for stabilized patients in the home setting and our
state-of-the-art ambulatory care Infusion suites.
We are the franchise of Vital Care Elmhurst which was established in 1986 and has a network of over
seventy-five providers located throughout seven teen states., Vital Care Elmhurst is able to provide prompt
service for patients. All services are centrally directed by Vital care Elmhurst's Coordination Center to the
Vital care Elmhurst. All services are rendered by Vital care Elmhurst, assuring the highest quality of care,
comfort and convenience to the patients and their families. This allows patients and their families'
immediate access to the provider for questions and concerns regarding the prescribed therapy and
treatment.
By utilization of highly professional staffs, response time for the initiation of therapies, changes
of therapies, the evaluations of therapies, and/or the correction of difficulties with therapies is
greatly reduced. Vital care Elmhurst provides answers to patients' concerns and provides on-going monitoring
on a clinical basis of patient therapies.
Vital care Elmhurst professionals can coordinate the provision of any ancillary services required by the
patients. As always, Vital care Elmhurst will monitor all services provided for cost-effectiveness,
professionalism, effectiveness of therapy, and quality of life.
Vital care Elmhurst's provision of these services will relieve referral sources of the responsibility
of coordination of all of the different services necessary for the proper provision of therapies.

not-for-profit corporation that has set high national standards for providers to meet in effectively and
efficiently delivering quality health care products and services to consumers. ACHC awards accreditation
on organizations that have demonstrated through rigid analysis that their policies, procedures,
practices and quality assurance programs meet the ACHC national standards. Accreditation is a voluntary
method to prove our commitment to continuously meet and exceed the high standards, to show that we are
in compliance with all regulatory bodies in the industry, and to provide a guarantee to any concerned
party that we strive for excellence in all endeavors.

ACHC reviewers perform on-site visits to audit these and other performance

entire body of evidence, they then determine whether or not we have met the standard sufficient for

providers in various specialties in each community we serve. We work in collaboration to
ensure an accurate, efficient, and positive patient experience.
What types of patients receive home infusion
therapy?
Referrals are quickly processed and benefits
are verified. Once checked out, an order is processed and appointments are scheduled for
in-home care or at our many convenient infusion sites.
Would home infusion be right for me?
Vital care Elmhurst provides treatment in the comfort
and privacy of a patient's home. This is often a more convenient and cost-effective
option.
What does a home infusion pharmacy do?
As your community home infusion pharmacy,
Vital care Elmhurst's responsibility is to prepare your medications under sterile conditions in
our preparation rooms, also called 'clean rooms', dispense all of the necessary supplies
that you and your nurses will need to give the medication, dispose of your medical waste
and provide professional pharmacy consultation on the use and monitoring of the
medication the doctor has prescribed. Many times our pharmacists, physicians, or
dieticians are involved in monitoring patient's lab results and suggesting changes in
therapy to your healthcare provider.
Will I have to give the medication to myself? Will
someone in my family have to do it?
There is amazing technology today that can make
even some of the most complex therapies easier to administer. Many patients are taught to give
their own medication, and learn to do it easily and safely.
However, in many situations nurses from a community home health agency are involved.
They come to your home as needed to administer the medication for you.
How do I make arrangements to get home infusion therapy?
There are several ways that you can get information
and possibly make arrangements to receive home infusion therapy.
Contact your doctor. Ask him or her if your medication can be given in your own home by a nursing
agency, or home infusion pharmacy.Contact Vital care Elmhurst at 270-506-2463 and ask to speak to one of our
corporate pharmacists or nurses. We can give you additional information needed to make your home care
arrangements.Contact a local home care agency, and ask them if they provide home infusion.
If they do, ask them if they can work with a Vital care Elmhurst Home Infusion Pharmacy to get your home
care started.
How do I select a provider?
When you are discharged from your hospital stay,
your physician may order additional therapies or equipment to be delivered to your home to aid
in your recovery. Homecare industry standards direct discharge staff to present you with an
unbiased list of potential suppliers in your area. You have the right to choose the provider
that you wish to continue your care in the home or alternate site. While we hope that Vital care
Elmhurst will be your choice of provider, we encourage you to exercise this right to remain in
control of your health care.
What does a home infusion pharmacy do?
Does insurance pay for home infusion therapy?
The answer depends on your insurance, your coverage plan, your medical condition,
and the medication that your doctor is prescribing. One of the services offered by
Vital care Elmhurst is to contact your insurance provider and get all the information we need
to decide whether or not your therapy would be reimbursed if administered in the home.
